  • Seven days' cancellation notice before the next billing date
  • Four, eight or twelve treatment credits per billing cycle, depending on membership
  • Unused treatment credits generally do not roll over
  • At least 24 hours' notice required for routine appointment rescheduling
  • Missed appointments and late cancellations may result in forfeiture of the associated credit
  • Medical exceptions permit cancellation and appropriate prorated refunds for unused prepaid treatments
  • Treatments require individualized medical approval

Your NAD+ injection membership automatically renews monthly until canceled. You may cancel without an early termination fee by providing notice at least seven days before your next billing date.
